Kindly read the following instructions carefully before filling up the form.

1. Enter mobile no., then click on Generate code option for password. Password will be sent to the same mobile no. within 2-3 minutes. This password will be used for login.

2. Fill up the form in CAPITAL LETTERS ONLY.

3. Please fill the form carefully. No correction will be allowed once submitted.

4. Keep details of Debit Card/Credit Card/Net banking/UPI ready for online payment of Registration Charges against Rs 1000/-.

5. Only the registration forms paid with Rs. 1000/- as registration fee, will be considered for further admission process.

6. Make sure to save the Registration number generated at the end of online Registration process. This is required for any kind of further communication with school regarding Registration /Admission of child.

7. If the filled registration form is not displayed on the screen after the payment, kindly e-mail your query at davpatiala.com mentioning the registration number.

8. Allocation of points for admission are auto generated by software. So, please provide accurate information while filling the registration form.

9.Incomplete /Incorrect/Duplicate Registration Form will not be considered.

Printout of the online filled Registration form, duly signed by both the parents, must be dropped in the drop box available at School Gate (No.1), along with self-attested photocopies of the supporting documents(as per the list given below) and passport size photographs of Child, Mother , Father and Guardians.

List of supporting Documents

1. Date of Birth certificate of child
2. Aadhar Card of child
3. Any one proof of Residential address
   a. Aadhar card/UID card of the parents
   d. Domicile certificate of the Child/Parents.
4. In case of First Born Child -Affidavit/Self Declaration by the parents.
5. Proof of Alumni of DAV Public School , Bhupindra Road, Patiala (if applicable),Photocopy of Certificate/Marksheet of last class Passed/School Leaving Certificate.
6. Proof of Sibling -Real Brother/Sister (if applicable) - Last fee paid Receipt and I card of the student.
7. Medical Fitness Certificate of the child.
8. Fee Once paid is not refundable.
